VTE Treatment

ACCP 2008
Treatment of Superficial Vein Thrombosis

Prophylactic or Intermediate dose of LMWH (2B) or intermediate dose of UFH (2B) for at least 4 weeks.
Alternative: 5 days of LMWH/Low-dose UFH + VKA for 4 weeks

Initial Treatment of DVT lower limb or PE Therapeutic dose of LMWH, UFH, Fondaparinux: 1A;
for at least 5 days : 1C
Patients with DVT or PE & cancer
  • LMWH for the first 3 to 6 months: 1A
  • Subsequently LMWH or VKA: 1C
Treatment of Upper Extremity DVT Therapeutic dose of LMWH, UFH, Fondaparinux: 1C
(then VKA)
