Drug details for : ZELAPAR
 
Drug class description :  Monoamine-oxidase-B inhibitors.
Generic Name :  Selegiline
Drug description :  Pale yellow oral lyophilisate (freeze-dried tablets) marked A.
Presentation :  Tablets (oral lyophilisates), selegiline hydrochloride 1.25 mg .
Indications :  Adjunct to levodopa (with a peripheral decarboxylase inhibitor) in the treatment of Parkinson's disease. Zelapar in combination with maximal levodopa therapy is indicated particularly in patients who experience fluctuations in their condition such as 'end-dose' type fluctuations, 'on-off' symptoms or other dyskinesias. As monotherapy in early Parkinson's disease for symptomatic relief and/or to delay the need for levodopa.
Adult Dosage :  Initially 1 lyophilisate allowed to dissolve on the tongue each morning before breakfast. Patients receiving 10 mg selegiline tablets can be switched to Zelapar 1 .25 mg.
Child Dosage :  Not applicable.
Contra Indications :  Known hypersensitivity (including severe dizziness or hypotension) to conventional selegiline tablets or liquid or any of the excipients used in this product. Patients receiving treatment with serotonin-agonists (e.g. sumatriptan). Phenylketonuria. Concomitant use with pethidine, other opioids and selective serotonin reuptake inhibitors. Should not be used in patients with other extrapyramidal disorders not related to dopamine deficiency. Active duodenal or gastric ulcer. Should not be used in patients who are being treated with antidepressant drugs or other monoamine oxidase inhibitors. Pregnancy and lactation. Selegiline in combination with levodopa is contraindicated in severe cardiovascular disease, arterial hypertension, hyperthyroidism, phaeochromocytoma, narrow-angle glaucoma, prostatic adenoma with appearance of residual urine, tachycardia, arrhythmias, severe angina pectoris, psychoses, advanced dementia and thyrotoxicosis.
Special Precautions :  Unstable hypertension, cardiac arrhythmias, severe angina, psychosis, history of peptic ulcer or hepatic impairment.
Interactions :  SSRIs and other antidepressants, pethidine or other opioids, non- selective MAOIs, sympathomimetics, nasal decongestants, hypertensive and antihypertensive agents, psychostimulants, CNS depressants, alcohol, amantadine, anticholinergics, digitalis, anticoagulants, serotonin agonists.
Adverse Reactions :  Back pain, dizziness, tremor, sore throat, stomatitis, mouth ulceration, impaired balance, muscle cramps, joint pain, insomnia, abnormal dreams, hallucinations, nasal congestion, pharyngitis. Depression, chest pain, myopathy, headache, tiredness. Transient increase in liver enzymes. Postural hypotension, involuntary movements (reduce levodopa dose).
