Drug class description :  5HT1-agonists
Generic Name :  Eletriptan
Drug description :  Round, film coated, convex orange tablets marked 'REP 20' or 'REP 40' on one side and Pfizer on the other side.
Presentation :  Tablets, eletriptan (as hydrobromide) 20mg, 40mg.
Indications :  Acute treatment of the headache phase of migraine attacks, with or without aura.
Adult Dosage :  Recommended initial dose, 40mg, taken as early as possible after onset of migraine headache. If migraine recurs within 24 hours, repeat dose after a minimum of two hours. If no response to first dose within two hours, a second dose should not be taken for the same attack. Maximum daily dose, 80mg.
Child Dosage :  Under 17 years, not recommended
Elderly Dosage :  Over 65 years, not recommended
Contra Indications :  Hypersensitivity to eletriptan hydrobromide or to any of the excipients. Severe hepatic or renal impairment. Moderately severe or severe hypertension, untreated mild hypertension. Confirmed coronary heart disease, including ischaemic heart disease (angina pectoris, previous myocardila infarction or confirmed silent ischaemia), objective or subjective symptoms of ischaemic heart disease, or Prinzmetal's angina. Significant arrhythmias or heart failaure. Peripheral vascular disease. History of cerebrovascular accident (CVA) or transiaent ischaemic attack (TIA). Administration of ergotamine or derivatives (including methysergide) within 24 hours before or after treatment with eletriptan. Concomitant administration of other 5HT1 - agonists.
Special Precautions :  For use only when a clear diagnosis of migraine is established. Not recommended for hemiplegic, ophthalmoplegic or basilar migraine, or for the treatment of atypical headaches. Not to be given without prior evaluation to patients in whom unrecognised cardiac disease is likely, or patients at risk of coronary artery disease (eg, patients with hypertension, diabetes or a strong history of CAD, smokers or patients using nicotine replacement products, men over 40 years of age, postmenopausal woman). Mild or moderate renal impairment; reduce dose (contraindicated in severe impairment). Pregnancy and lactation. Not to be given together with potent CYP3A4 inhibitors; see Interactions.
Interactions :  Ergotamine, ergot derivatives (including methysergide), other 5HT1-agonists; concomitant use contraindicated. Potent CYP3A4 inhibitors such as ketoconazole, itraconazole, erythromycin, clarithromycin, protease inhibitors (ritonavir, indinavir, nelfinavir). St John's Wort (hypericum perforatum).
Adverse Reactions :  Most commonly, asthenia, somnolence, nausea and dizziness. Also chest symptoms (pain, tightness, pressure), headache, abdominal pain, back pain, chills, throat tightness, sensation of warmth or flushing, palpition, tachycardia, dry mouth, dyspepsia, myasthenia, myalgia, tingling or abnormal sensation, sensation of tightness or stiffness, hypoaesthesia, vertigo, pharyngitis, sweating.
