Drug class description :  GP IIb/IIIa inhibitors (antiplatelet agents).
Generic Name :  Abciximab
Drug description :  Solution for injection in vial.
Presentation :  Injection, abciximab 2mg per mL.
Indications :  Adjunct to heparin and aspirin for: The prevention of ischaemic cardiac complications in patients undergoing percutaneous coronary intervention (balloon angioplasty, atherectomy and stent). The short-term (1-month) reduction of the risk of MI, in patients with unstable angina, not responding to conventional therapy who have been scheduled for percutaneous coronary intervention.
Adult Dosage :  Recommended dose, 0 .25 mg/kg by bolus intravenous injection immediately followed by a 0.125 microgram/kg/minute (to a maximum of 10 microgram/minute) continuous intravenous infusion. For stabilisation of unstable angina, the bolus dose followed by the infusion should be started by the infusion should be started up to 24 hours prior to the possible intervention and concluded 12 hours after the intervention. For the prevention of ischaemic cardiac complications in patients undergoing percutaneous coronary intervention, and who are not currently receiving a ReoPro infusion, the bolus should be administered 10 to 60 minutes prior to the intervention, followed by the infusion for 12 hours. Administer oral aspirin at a dose of not less than 300 mg daily. Administer heparin in a low-dose weight adjusted regimen; see Summary of Product Characteristics.
Child Dosage :  Not recommended.
Elderly Dosage :  Over 80 years, not recommended.
Contra Indications :  Sensitivity to abciximab or murine monoclonal antibodies. Active internal haemorrhage, history of cerebrovascular accident within last 2 years, recent (within two months) intracranial or intraspinal surgery or trauma, recent (within two months) major surgery, intracranial neoplasm, arteriovenous malformation or aneurysm, bleeding diathesis, severe uncontrolled hypertension, pre-existing thrombocytopenia, vasculitis, hypertensive or diabetic retinopathy, severe hepatic or renal failure. Previous monoclonal antibody therapy. Lactation.
Special Precautions :  Administer under specialist care only. Incidence of bleeding reduced by use of low-dose weight adjusted heparin, early sheath removal and improved patient and access site management; see Summary of Product Characteristics. Risk of anaphylaxis; cardio-pulmonary resuscitation facilities must be on-hand. Perform full blood tests 2 and platelet counts before and after therapy; transfuse platelets if count drops below number of potential bleeding sites; pretreat with an H - antagonist to prevent GI bleeding. Renal and peripheral vascular disease. Pregnancy.
Interactions :  Additive effects with thrombolytics, anticoagulants, antiplatelet agents.
Adverse Reactions :  Bleeding complications, hypertension, nausea, vomiting thrombocytopenia, haematoma, bradycardia, fever, vascular disorders.
