| Drug description : | Klassifisering/Virkningsmekanisme: Kraftig hemmer av xantinoksidase, enzymet som omdanner hypoxantin til xantin og xantin til urinsyre, som er sluttproduktet i purinstoffskiftet. Allopurinol reduserer urinsyrespeilet både i serum og urin. |

TABLETTER 100 mg og 300 mg: Hver tablett inneh.: Allopurinol. 100 mg, resp. 300 mg, lactos. 6 mg, resp. 18 mg, const. ad 180 mg, resp. 540 mg. Tabletter 100 mg med delestrek. |
| Indications : | Arthritis urica, spesielt med tophi, varige leddforandringer, samt profylaktisk ved hyppig residiverende anfall. Andre tilstander med hyperurikemi. Behandling og profylakse ved gjentatte tilfeller av kalsiumoksalat-/fosfatnyrestensanfall hos pasienter med hyperurikemi og hos pasienter med urinsyrenefropati eller med tendens til å danne uratstener. Strålebehandling eller bruk av cytostatika kan medføre så uttalt hyperurikemi at det opptrer nyreskade med nyresvikt, ev. anuri. Allopurinol kan forebygge ev. behandle urinsyrenefropati hos disse og andre pasienter med økt cellehenfall og nukleoproteinnedbrytning. |
| Adult Dosage : | Podagra: Vanligvis 200-300 mg daglig. Pasienter med svær urinsyregikt med tophi kan trenge inntil 600 mg/dag. Normalt urinsyrenivå i serum oppnås vanligvis på 1-3 uker. Profylakse og behandling av uratstener: Samme doser som anbefalt ved urinsyregikt, sammen med høyt væskeinntak og alkalisering av urinen. Forebyggelse av urinsyrenefropati ved maligne sykdommer: Behandlingen bør startes med 200 mg 3 ganger daglig 3 dager før strålebehandling eller behandling med cytostatika påbegynnes. Bør gis så lenge en kan vente hyperurikemi. Inntil 300 mg kan gis i en dose. (Se Bivirkninger.) Barn: 6-10 år: 100 mg 3 ganger daglig, under 6 år vanligvis 50 mg 3 ganger daglig. Responsen bør vurderes etter 48 timer og dosen justeres om nødvendig. Dosering ved nedsatt nyrefunksjon: Dosering ved dialysebehandling: Allopurinol og dets metabolitter fjernes under dialysebehandling. Om dialyse er påkrevd 2-3 ganger pr. uke bør en alternativ dosering vurderes, med 300-400 mg allopurinol etter hver dialyse. Ingen dosering mellom dialysebehandlingene. |
| Child Dosage : | Hemmer av urinsyreproduksjonen. |
| Contra Indications : | Overfølsomhet for allopurinol. Graviditet. |
| Special Precautions : | Initialt kan behandling med allopurinol fremkalle anfall av arthritis urica. Det anbefales at det profylaktisk gis kolkisin (0,5 mg × 3) den første måneden av behandlingen. Ved nedsatt nyrefunksjon bør dosen av allopurinol reduseres pga. faren for kumulasjon. |
