Drug class description :  ACE inhibitors / thiazide diuretics (thiazides).
Generic Name :  Quinapril, hydrochlorothiazide
Drug description :  Pink scored elliptical f-c tablets.
Presentation :  Tablets, quinapril 10mg, hydrochlorothiazide 12.5mg.
Indications :  Treatment of all grades of essential hypertension in patients who have been stabilised on the individual components given in the same proportions. given in the same proportions.
Adult Dosage :  One tablet daily; maximum two tablets daily. Take without food.
Child Dosage :  Not recommended.
Contra Indications :  Pregnancy, lactation. Patients with anuria or hypersensitivity to quinapril, thiazides or any sulphonamide derived drug. History of angioedema related to previous treatment with ACE inhibitors. Hereditary/idiopathic angioneurotic oedema.
Special Precautions :  Sensitivity reactions may occur in patients with or without a history of allergy or bronchial asthma. Severe renal disease. Hepatic impairment, severe congestive heart failure, renal artery stenosis, haemodialysis, collagen vascular disease, diabetes, surgery/anaesthesia. Monitor renal function, white cell count, BUN and urinary protein before and during therapy.
Interactions :  Tetracyclines, K+-sparing diurerics, K +-supplements, other diuretics, anaesthetic agents, tubocurarine, lithium, corticosteroids, ACTH, NSAIDs, allopurinol, cytostatic and immunosuppressant agents, procainamide, alcohol, barbiturates, narcotics, other antihypertensives, antacids or food, antidiabetic drugs.
Adverse Reactions :  Most frequently, headache, dizziness, rhinitis, cough, fatigue. Also, myalgia, viral infection, nausea and vomiting, abdominal pains, back pain and upper respiratory infection. Less frequently, dyspepsia, chest pain, diarrhoea, insomnia, bronchitis, somnolence, asthenia, pharyngitis, vasodilation and vertigo. Thiazide effects. Adverse events seen only occasionally in clinical trials and post-marketing experience included: tachycardia, palpitations, syncope; flatulence, dry mouth or throat; dyspnea, sinusitis, bronchospasm; dermatitis, alopecia, pemphigus, pruritus, rash, Stevens Johnson syndrome; paresthesia, nervousness; impotence, urinary tract infection; arthralgia, peripheral edema, hemolytic anaemia. The following side effects have been observed associated with ACE Inhibitor therapy: myocardial infarction, transient ischaemic attacks, cerebral haemorrhage; cholestatic icterus, hepatitis and ileus, rarely, pancreatitis; urticaria, epidermic necrolysis, psoriasis like efflorescences; rarely, depressions, confusion, tinnitus, blurred vision and taste disturbances; decreases in hematocrit, platelets and white cell count as well as elevation of liver enzymes and serum bilirubin.
