INDERETIC

• Drug Class Description
  Non-cardioselective b-blockers (beta-blockers) / thiazide diuretics (thiazides).
• Generic Name
  Propranolol, bendrofluazide [bendroflumethiazide]
• Presentation
  Capsules, propranolol hydrochloride 80mg, bendrofluazide 2.5mg.
• Description
  White capsules marked INDERETIC.
• Indications
  

  Hypertension.

• Adult Dosage
  1 twice daily.  1 twice daily.
• Child Dosage
  Not recommended.
• Contra Indications
  

  2nd or 3rd degree AV block, sinus bradycardia, sick sinus syndrome, after prolonged fasting, severe peripheral arterial disease, uncontrolled heart failure, cardiogenic shock, hypotension, untreated phaechromocytoma.

  Non- cardioselective b-blockers are also generally contra-indicated in patients with obstructive airways disease or a history of bronchospasm. Anuria, severe or progressive renal failure. Known hypersensitivity to either component. Prinzmetal's angina. Metabolic acidosis.

• Special Precautions
  

  Poor cardiac reserve should be controlled with digitalis and diuretics. Portal hypertension. Diabetes; metabolic acidosis. Asthenic symptoms. Cerebrovascular insufficiency. Renal or hepatic impairment, thyrotoxicosis.

  General anaesthesia; consider withdrawal before elective surgery. Pregnancy, lactation. Withdraw gradually. Gout, diabetes, hyperlipidaemia. Monitor electrolyte levels; thiazide diuretics can cause hypokalaemia so K +-supplements may be required.

• Interactions
  

  Clonidine withdrawal, verapamil, diltiazem, class II antiarrhythmics, cardiac depressant anaesthetics, hypoglycaemics, reserpine, other antihypertensives, ergot alkaloids, cimetidine, rifampicin, theophylline, warfarin, CNS depressants, sympathomimetics, indometacin (indomethacin), ibuprofen. Lithium, NSAIDs, digitalis, allopurinol, amantadine.

• Adverse Drug Reactions
  

  Cold extremities, sleep disturbances, bradycardia, exertional tiredness, bronchospasm, heart failure deterioration, postural hypotension, GI upset, alopecia, precipitation of heart block, claudication, Raynaud's phenomenon, confusion, dizziness, mood changes, nightmares, psychoses and hallucinations, purpura, thrombocytopenia, alopecia, dry eyes, psoriasiform skin reactions, exacerbation of psoriasis, skin rashes, paraesthesia, visual disturbances, gout, photosensitivity, rash, blood dyscrasias, weakness, fatigue and/or lassitude, increase in antinuclear antibodies.
