Gastroenterology Drug Data - A-Z (English)

Drug Class Description

Proton pump inhibitors (PPIs) / macrolides / broad spectrum penicillins (antibiotics).

Generic Name

Lansoprazole, clarithromycin, amoxicillin [amoxycillin]

Drug Description

Zoton Capsules: lilac/purple capsules marked with strength and company name. Klaricid Tablets: yellow oval f-c tablets with logo. Amoxycillin Capsules: capsules.

Presentation

Treatment pack containing 14 Zoton (lansoprazole) capsules, 14 Klaricid (clarithromycin) 500mg tablets, 28 amoxicillin 500mg capsules.

Indications

Eradication of Helicobacter pylori in patients with duodenal ulcers

Adult Dosage

1 x Zoton capsule, 1 x Klaricid tablet and 2 x amoxycillin capsules all twice daily for seven days.

Child Dosage

Not recommended

Contra Indications

Severe renal impairment. Pregnacy

Special Precautions

Exclude malignacy pror to treatment. Renal or hepatic treatment. Renal or hepatic impairment. Lactation

Interactions

Drugs metabolised by cytochrome P450 system (eg, warfarin, ergot alkaloids, triazolam, midazolam, disopyramide, lovastatin, rifabutin, phenytoin and cyclosporin). Rhabdomyolysis, coincident with the co- administration of clarithromycin, and HMG-CoA reductase inhibitors, such as lovastatin and simvastatin has been reported.

Theophylline, oral anticoagulants, digoxin, carbamazepine, ritonavir, zidovudine, statins, phenytoin, antacids, sulcrafate, oral contraceptives.

Adverse Reactions

Amoxycillin : GI upset, hypersensitivity reactions. Clarithromycin : GI upset, paraesthesia, stomatitis, glossitis, oral monilia, tongue and tooth discolouration, headache, arthralgia, myalgia, and allergic reactions ranging from urticaria and mild skin eruptions and angioedema to anapyhlaxis and rarely Stevens- Johnson syndrome.

Hepatic dysfunction, reversible hearing loss. Rarely, pseudomembranous colitis, hypoglycaemia, leucopenia, thrombocytopenia, renal dysfunction, interstitial nephritis, and renal failure. QT prolongation, ventricular tachycardia and Torsades de Pointes have been reported rarely.

Lansoprazole: Headache, dizziness, fatigue, malaise, GI upset, sore throat, skin irritation, hypersensitivity reactions, raised liver function values, jaundice, hepatitis, increased risk of GI infections. Rarely arthralgia, myalgia, peripheral oedema, haematological changes, erythmatous or bullous rashes, photosensitivity, interstitial nephritis, confusion, hallucinations, gynaecomastia, impotence.
